The Rancho Santa Fe School District would like to thank all the families that have returned their child’s Registration Packet thus far.
If you have not had a chance to turn in a Registration Packet for the 2010-2011 school year, please review the following directions and return your Registration Packet during the hours of 7:30 – 4:00 this week (August 23 - 27).
1. Carefully review your Emergency Card (one per student), lining through any information that is not correct and writing in the current information so we can update our student information system.
2. Review the Family Guide with your child/children and complete the Signature Form (One Per Student). Families with more than one child may download additional forms below (in English or Spanish) as needed. If you prefer, you may Xerox copies of the Signature Form included in your Registration Packet. Please be sure to complete both front and back pages of this form.
3. Included is a Confidential Health Form to complete if your child has had any changes in their health, or if they are in 4th or 7th grade.
4. In addition to the required forms above you will also find the following optional forms to complete:
· Private Vehicle Transportation Form: complete and return with your insurance policy affidavit form attached proving dollar amount of coverage (this is required for parent drivers for classroom and athletic field trips).
· Yearbook Order Form with yearbook payment attached.
· Educational Foundation Donation Form and red envelope to use for your donation.
Please place the completed forms in the Registration Packet envelope and bring them to the school office prior to the first day of school (Tuesday, September 7).
